Greetings, Our association has a very simple dues letter - no date of when the form was sent or a date when payment is due.  Does anyone have any sample dues letters/forms they would be willing to share?

I notice that your sample form refers to the formula in your bylaws, but you do not say that a copy of the bylaws is attached.  Could you explain how your formula works?  Thanks!

I'll attach another possible format for an invoice.  We send out the Minutes of our Annual Meeting and our Financial Report along with the bills
